JOB DESCRIPTION: Social Worker (MSW) Per Diem Position Summary :
Provides initial and ongoing assessments for patients, families, counseling , crisis intervention ,consultation to medical staff re: patients psychosocial problems/discharge needs referrals to appropriate community resources, attending hospital and community committees, participation in discharge planning, inservices, professional organizations, keeping statistics, assistance with QA/QI.
**This is a Per Diem position; apply accordingly.
JOB REQUIREMENTS: Experience :
One year post graduate experience.
Education :
Masters Degree in Social Work from an Accredited College.
Required License / Credentials :
LSW or LCSW

Supervision Given :
Provides supervision to undergraduate and graduate social work interns as needed.
Supervision Received :
Performs duties independently, following established procedures and reporting periodically to immediate supervisor. Receives clinical supervision on self-identified cases.
