This position requires managing an assigned case workload. Review files,interprets medical reports, and makes recommendations for promoting appropriate cost effective outcomes. Performs some medical case management hours of operation for this position is Monday - Friday 8:15 am to 4:30 pm. This position is located in our Farmingdale, NY office.
Responsibilities: * Contacts providers as needed to determine the appropriateness and duration of treatment. * Reviews files to determine the need and timing for IMEs and Peer Reviews. Identifies the appropriate medical specialty and recommends questions to be asked at the time of review. * Educates Personal Market staff, Claims Representatives and Claims Specialist on case management and medical terminology. Interpretation of Medical procedures, Diagnostic testing, IME, and Peer Reviews. * Identifies the need, make recommendations and when requested, arranges, for attendant care, durable medical equipment, prescription drugs, transportation, and specialty facility admissions. Refers to preferred providers when possible. * When requested by the case owner, performs in person assessment and communicates with the physician to establish diagnosis, treatment plan and prognosis on complex/catastrophic cases. Develop and Implements individualized case management plan. In person assessment on older cases to determine status and on going need for treatment and services. * Communication with case owner on a regular basis. * Documentation in Workbench of all activity performed, recommendations and cost savings. Monthly completion and submission of time tracking logs to include time, travel and miscellaneous expenses on each assigned case. * Undertake assignments at the direction of the On Site Nurse Manager. Qualifications: * 3 years of relevant nursing experience (BSN preferred) * Must have current registered nurse license as required by law in the state of NY and/or MI. * Prefer additional certifications such as, C.R.R.N., C.I.R.S. and C.C.M. where required by law. * Ability to analyze and make sound nursing judgments is required. * Prefer knowledge of state, local and federal laws related to health care delivery. * Michigan no fault state law is a plus. * Strong interpersonal communication skills required. * Demonstrated ability to accurately document activities is required. * Prefer 1-2 years of computer experience. * Requires occasional travel in order to conduct in persons assessments and meeting with medical providers.
